Your First Visit

We strive to ensure all of our new patients feel welcome, safe and taken care of. During your first visit our dental team will review your dental and medical history. For diagnosis we will require dental x-rays prior to examination and cleanings. This helps us provide you with the best care and treatment plan possible. Our team of dentists will work with you to establish a treatment plan and if it is convenient for you, we will schedule you for a cleaning too!

Please bring a printed copy of the completed forms to our clinic upon the date of your appointment. You can fill most of the information in directly on the PDF files before you print them if you wish. Due to security reasons, we will not accept any forms submitted by email at this time. Thank you.

Insurance Guidelines

As a courtesy to you, we will direct bill your insurance if allowed by your insurance company. Please bring your insurance card and information with you to your first visit.
